This contract is attached to my high school art syllabi. I've used the same contract in middle school. I have students keep this in their art folders so that if I ever need to address any of these issues with them, we can first visit the contract and note that they read the items, checked them off and signed the document. I also have parents sign this so that I can side-step any misunderstandings about student responsibility and behavior and my expectations.
