The following forms are what I use for art club. The first permission slip is the one I send home to parents when I start art club and choose the students who can participate. The second form is my permission slip I use each year to go on our field trip. I include an itinerary of what we are doing, a checklist of things to bring and not bring, and the permission for that needs to be filled out. This form will change based on the field trip, but the attached form is the one I used last year when we went out of the U.S. and into Ottawa, Canada. The third form is the form I send home at the time of the art show. My art club student usually spend the afternoon at school giving tours to the community of our art show as they come in to vote on the budget.
