Great rubric that specifically records for parents, the behavioral concerns you have for their child in the art room. It is written in a reflective, helpful, positive and approachable manner. I can attach this form to email, sent through mail, send home with student or share during Parent-Teacher conferences. What's so great about this form is that it pinpoints the behavior that is hold their child back from being successful in the art room AND it identifies successful strategies that can be employed and practiced at home and in school to increase the likelihood that a student will be able to meet with success. This form can be adapted to any grade (although, as is it is applicable to upper elem. to high school. And it is very concrete and helpful to students with and learning or behavioral disability.)
